Knowing All About Wisdom Teeth

The wisdom teeth or the third molars are known to the last ones to come out. These new teeth can cause problems as they erupt inside your mouth. This is because it will present a change in your dental structure. These alterations can cause you severe discomfort and pain and therefore you need to have enough knowledge about how the process works in order for you to understand why you need to remove the teeth.

Symptoms of Impacted Wisdom Teeth

If you are in late teens or early twenties and feel pain and difficulty in opening your mouth and have swelling and headache then the most probable cause is that you have impacted wisdom tooth trying to grow in. The wisdom tooth comes after your last 12 year molar. Because of decreasing size of jaw due to evolution the problem of impacted tooth is more as they do not get enough space in jaw to grow in and when they try to grow in gums they cause severe pain and swelling in gums. Mainly the pain and swelling is there when the wisdom teeth get infected. When it is infected one can have following sign and symptoms

Pain in Gums – Pain in gums is due to infection when the wisdom teeth try to come in the mouth and there is lack of space in gums. Initially in early stages it can start as dull pain but within few days it can turn into acute pain. This wisdom tooth pain can even be sometimes felt as ear ache, headache, difficultly in swallowing and other adjacent teeth can be painful. There can be pain in opening the jaw. You can also feel lethargic and you may feel feverish.

Swollen and Inflamed Gums – As the wisdom tooth is trying to break in through the gums, the gums become swollen and red causing acute pain. There can be bleeding also from gums. Due to gum pain in that area, there is difficulty in swallowing.

Swelling in Jaw - The jaw swelling can also happen due to infection of gums which is basically due to either less space for eruption of wisdom tooth or due to wrong angle of growth of wisdom tooth. This swelling of jaw is very painful and patient is unable to open the mouth.

Bad Taste in Mouth – One can feel very bad taste in mouth if there is impacted wisdom tooth trying to grow in. This bad taste is due to infection in the gums and along with this as there is inability to open the mouth, the patient is not able to maintain very good oral hygiene. Brushing becomes difficult.

Bad Breath – Infected wisdom tooth causes pain, swelling and infection of gums and due to infection there is pus in the gums. This infection causes bad breath along with unpleasant taste in the mouth.

Headache – Headache is mainly a referred pain and it is due to pain and swelling in gums. Because of pain and swelling the patient is not able to open the jaw and experiences headache on that side where the impacted wisdom tooth is trying to grow in.

Crowding and Decay of Teeth – Impacted wisdom tooth can cause the decay of other teeth especially adjacent molars as it is difficult to clean that area and food impaction is there. This impacted wisdom tooth can cause crowding also as they push the other teeth to create space for themselves.

Failure of Orthodontic Braces Treatment – If you have undergone the braces treatment and the wisdom tooth are not taken into consideration then this can cause failure of your braces treatment when wisdom teeth try to break in. So normally due to this the wisdom teeth are extracted before the start of braces treatment.

It is not always that wisdom teeth causes problems but if they are impacted they can be really dreadful and if ignored they can cause several complications ranging from gum diseases, decay and damage of other teeth. So getting the treatment for wisdom teeth is very important and the treatment is extraction of wisdom teeth.

Wisdom Tooth Extraction Surgery, Healing and Postoperative Care

Surgical Extraction of Impacted Wisdom Teeth

Surgery is usually held in your local dental clinic rather than the hospital. Your dentist will ensure that he or she explains the procedure very carefully and with as much tact as possible. The doctor will discuss about the healing process, the operation itself and other information you might want. The use of anaesthetics will ensure that you feel no pain because dentists might have to cut lesions in your mouth to perform the operation.

Healing Process after Surgical Extraction of Wisdom Teeth

The healing process immediately begins after the operation because the blood will rush back into the socket to provide the necessary nutrients for clot formation. A gauze will provide the sufficient pressure necessary for immediate clot formation.

To further promote healing, you must follow the doctor’s prescriptions and advice regarding postoperative medication. You must also go back for follow up consultations. Furthermore, the moment you feel any pain or swelling during recovery, you must immediately go to the dentist for consultation.

Care after Surgical Extraction of Wisdom Tooth

In order for your teeth to heal properly after the removal, you must be observant as to any aberrant signs of bleeding or swelling during the period. The eight hours right after the operation are found to be the most critical but otherwise, it is manageable.

After the operation, you may feel small bone parts scattered across your tongue. Those are the remnants of the tooth, which have been removed. It will disappear as soon as the gum heals. In addition, the stitches can also give you a little discomfort. You have to come back to the dentist for thread removal. However, if dissolvable threads were utilized, it will eventually melt on its own after a few days. Here are additional advices on recovery.

  1. Never Touch the Wound

Within the first 24 hours, be sure not to have physical contact with the wound. This may infect the spot. Make sure to use the unaffected side of your mouth for chewing.

  1. Smoking is prohibited for a Day

Smoking immediately will cause the wound to bleed.

  1. Bleeding Control

Be sure to have the gauzes replaced every so often if the wound had not been stitched up. It is important to apply pressure on the wound until clot formation.

  1. No Spitting or Sucking on Straws

This is another cause for bleeding, which can affect the clot and reopen the wound.

  1. Anti Pain Medication

Anti-inflammatory drugs are used to prevent discomfort after minor surgery. The dentist may prescribe stronger analgesics when the patient feels extreme pain.

  1. Swelling

During the first 24 hours, it is advised to put a cold compress on to the wound when swelling occurs with 20-minute intervals. It is also advisable to rinse your mouth with warm water and a teaspoon of salt after twenty-four hours to clean it. Do not do this before the allotted time so as not to disturb the clots.

  1. Recommended Diet
  • Soft
  • Avoidance of hot and cold drinks as well as food to prevent bleeding
  • Avoidance of alcohol

Schedule a visit to your dentist once a week to prevent any complication.

Esther, Pain can be because of swelling or inflammation of gums and surrounding soft tissues around partially erupted tooth. Flap of gum tissue could be there in partially erupted tooth where food impaction can occur. This condition is more common in lower wisdom teeth. Gums can become inflamed and swollen in this condition and will appear red. Pus discharge may also occur from gums. Condition is extremely painful and can also lead to bad taste and odor from mouth. Without clinical examination, it is difficult to give definite diagnosis. You can visit your dentist for evaluation. Debridement of area can be done for pain relief. If condition recurs, then operculectomy that is removal of flap tissue on partially erupted tooth can be done. By the time you visit a dentist, you can do warm saline rinses for some relief. Maintain a good oral hygiene to avoid any food accumulates. Food accumulates in relation to erupting wisdom tooth will aggravate the pain. You can also apply orajel over site for some pain relief and can take otc pain meds if pain is bad.

My lower left wisdom tooth has already grown in, maybe not fully but it's definitely more out then in and it seems to be aligned properly. I had the tooth right in from it extracted about a year ago, so theoretically there isn't anything it could get impacted in. However, I've have been feeling this terrible pain in the last few day, similar to the one I had in the case of an other impacted wisdom tooth I had before and it's only getting worse and worse. At the beginning it was only in the wisdom tooth itself, but now it seems to have migrated to the gum before it (the place of the extracted tooth) and the tooth before that. Do you think that all this is just caused by the wisdom tooth growing in or there might be something else behind it? Thanks

Alexa, You have not mentioned that when did you get your wisdom teeth removed. Some pain after wisdom teeth extraction is normal. Degree of pain and swelling after wisdom teeth extraction is dependent on amount of trauma to the investing tissues during extraction. It may take weeks to months for wisdom tooth extraction socket to heal completely depending on positioning of wisdom tooth and method used for wisdom tooth extraction. Pain in all teeth can be because of teeth clenching. You can get a mouth guard if you are in a habit of teeth grinding. Mouth guard relaxes the jaw muscles and avoids any teeth damage.

I had my 4 wisdom teeth extracted under general and I have been having problems falling asleep mainly because I find myself clenching my teeth together as soon as I start to nod off which makes we wake up with a jolt as it it is quite painful. I am not sure what to do to stop this as I have no control over it! I also have pain in my nose and my other teeth, is this normal?

Dom, If teeth are impacted in the bone, then bone cutting is required. You don’t have to worry about tooth extraction. Procedure will be done under local anesthesia or conscious sedation. There will be no pain during procedure. You will just feel some pressure but no pain. Surgical tooth extraction is done for impacted wisdom teeth. After numbing you, dentist will give cut on gums and if tooth is impacted in bone, then bone cutting is done. Tooth sectioning may also be required to remove the tooth. After tooth is taken out, stitches will be given to you to approximate flap margins. You will be given some instructions after teeth extraction which you need to follow to avoid any complications. To reduce pain after teeth extraction, pain meds will be given to you.

Hello im getting 3 of my wisdom teeth out on Friday. My right top one has already grown and is perfectly aligned but its longer then all my other teeth. My bottom 2 are impacted and will require extraction. How can I tell by my x-ray photo if bone cutting is required? Im really nervous about the procedure (more should I say the recovery) and would like to know as much as possible about whats going to happen. Thanks!

Paul, Mild pain after 2 weeks of difficult tooth extraction is normal. In case of difficult or surgical tooth extraction, it may take 3 weeks to 3 months for extraction socket to heal completely. Pain will regress in some time. If pain seems to aggravate, then you can visit your dentist for evaluation. Continue with warm saline rinses for faster healing of tooth extraction socket and follow the instructions of your dentist. Keep tooth extraction socket area clean to avoid any secondary infections.

Hello. I had three wisdom teeth extracted two weeks ago. All seemed well, but I notice that I still have some pain in the bottom wound. The pain is not intense. The wound looks fine, but when I touch it with my tongue it feels very hard. Is it normal to have some mild pain two weeks later? I'm told the extraction was a difficult one. Thank you.
Posted By : Dr Ritz on 1/23/2012 1:35:34 AM
Bryana, Pain can be because of healing of tooth extraction socket. If it was a surgical or complicated tooth extraction, then pain can be there because of normal healing. If it was a simple tooth extraction and pain is getting worse, then it can be because of dry socket. Dry socket occurs when blood clot is dislodged or it dissolves and it causes severe pain which may also be radiating. Stitches are given to approximate the flap margins and should be there for 7-10 days after tooth extraction. If stitches fall on day of tooth extraction, then resuturing can be done by the dentist. If pain doesn't seem to get better, than you can visit your oral surgeon for evaluation. Take the meds as prescribed and follow the instructions of your dentist.

Hello, I had my wisdom teeth removed on Thursday, it's Sunday now and the pain seems to increase. The pain is not only knnmy lower jaw, but more concentrated on my back left molar. That tooth was right next to the wisdom tooth that was goin in at a 45 degree angle towards my tooth. The gum also lost it's stitching the first night, and it's an open hole. I've been rinsing it withsalt water, but I can't seem to get relief? Should I call my surgeon? And if I did, would they be able to prescribe more pain medication?
Posted By : Dr Carl on 1/9/2012 2:08:20 AM
Vincent, Healing after wisdom tooth extraction is usually complete in 3 months after tooth extraction and pain is usually not there 3 months after extraction. Delayed pain 3 months after extraction can be there because of referred pain due to some other infected tooth present or can occur if the holes of extraction socket have not closed and food accumulates have occurred in tooth extraction socket. If there is some other infected tooth present, then you can get treatment done for the infected tooth. If there are food accumulates present, then you can brush over that area to remove them and can do warm saline rinses. If pain doesn't regress, then you can visit your dentist for evaluation.

Lee, Pain can be because of wisdom tooth eruption. Wisdom teeth are last teeth to come in mouth and causes pain which may be radiating to head and neck if they are impacted. Impacted wisdom tooth eruption also causes swelling of jaws and gums. Pain can also be because of deep caries on back tooth or can be there if a person is in habit of clenching or grinding his teeth. Your boyfriend can visit a dentist for dental examination. Dentist may also take an x-ray to see whether wisdom teeth are coming or not. If wisdom teeth are impacted, then they can be pulled out. If teeth grinding is reason of pain, then mouth guard will be given and if there is deep caries present in some tooth, then restoration or root canal treatment is done for the tooth.

My boyfriend is 25 years old and currently experiencing pain in the back-left portion of his lower jaw. This jaw pain is sometimes accompanied by toothaches and headaches. These bouts of pain usually last 2-4 days, then subside for a week or two before returning. Could this be indicative of a wisdom tooth problem?
Posted By : Dr Ritz on 11/7/2011 12:03:25 AM
Deborah, Yes it can be a wisdom tooth. Wisdom tooth usually erupts in late teens but can also erupt in age of 30-40 years. Wisdom tooth might not be there 10 years back and might have grown in the mean time. There might be roots of the loose tooth present underneath which you pulled out yourself. You can visit your dentist and get the tooth examined. He will take an x-ray to see whether it is a wisdom tooth or are the roots of other molar. If it is the roots of tooth or impacted or infected wisdom tooth, then it can be pulled out. By that time, you can do warm saline rinse to reduce the swelling of the gums and make sure that there are no deposits in that area which can aggravate the pain.

I am 47 and I lost the back bottom molar a couple of days ago after it had been wobbly and loose for a few months. I didn't go to the dentist - I just worked it out myself and it all seemed OK. However, now that the gum is settling down, I can feel what seems to be a wisdom tooth underneath and to the rear of where the molar had been. I am sure there were no wisdom teeth in the last X rays I had, about ten years ago! Could it have grown since then? Is this normal?
Posted By : Dr Ritz on 9/28/2011 12:29:40 AM
No George, It is not normal to have balance impairment and weird pain in head after 3 days of wisdom tooth extraction. It can be because of the pain meds which must be given by your oral surgeon after tooth extraction or can be because of TMJ problem after wisdom tooth extraction. You have to refer to your oral surgeon for this. Without clinical examination, it is difficult to comment. If it is because of pain meds, then get them changed. Its better if you take rest today and avoid work. You can do warm saline rinses and follow the instructions of your oral surgeon to avoid any secondary infections.

Musa, wisdom teeth if impacted can cause pain which may be radiating to the ear and neck. It causes swelling of the jaws and bad taste and bad breath from mouth. Impacted wisdom teeth can also cause headache if impacted.

Can these teeth give pain in all the body
Posted By : Dr Ritz on 7/20/2011 8:54:47 AM
If the tooth is fully erupted but wrong angled even then it can create problems but as you are leaving in 5 days, the decision to get it extracted depends on clincal and radiographic examination. If the roots are straight and it is normal extraction without any bone cutting then you can get it extracted but still the healing will take longer and chances of infection are always there. So it is better to avoid if it is not giving much problems and you can pass your trip with sos medicines. The healing usually takes around 1 week to 10 days in case of non surgical extraction and it can take 3 weeks to 3 months in case of surgical extraction depending on amount of bone cutting.
