Swelling of gums and sensitivity in tooth with dental crown

I have decay on the side of my tooth below the gum line. My dentist has never seen or heard of this before. It is sensitive and the gum is slightly swollen. I brush it to help relieve the swelling. What can be done besides extraction and implant? I have a crown on one side and the other tooth is my last one before the wisdom pocket. By the way the tooth in question is a crown also.

Swelling of gums can be there because of food accumulates and deposits around crowned tooth or can be there because of infection in the tooth. Plaque and calculus deposits around tooth structure can cause inflammation and the gums around tooth may recede. Gum recession or infection in the tooth can lead to tooth sensitivity. Your dentist can take an x-ray to see whether the tooth is infected or not. If the tooth is infected, then root canal treatment will be required for the tooth and if the tooth is already root canal treated, then re-root canal procedure will be required to remove the infection from the tooth. If swelling is because of plaque and calculus deposits around tooth, then brush and floss regularly to avoid any deposits around teeth. You can also do warm saline rinses 2-3 times a day. If there is small decay present on the side of tooth below gum line, then it can be restored with composite or glass ionomer filling material which are tooth colored filling material. If there is much of decay of cervical portion of tooth, then tooth extraction may also be required. In case of tooth extraction, the missing tooth needs to be replaced with dental implants or dental bridges. Without clinical examination, it is difficult to give definite answer.
