Hello!! My name is Hasib Ahsan. I'm 40 years old. Recently I felt acute pain in my tooth and later on I have been detected with carriage in my third moral (wisdom teeth)in my lower jaw of right hand side. The third molar is completely erupted. Seeing the dental x-ray and examining physically my dentist told me to extract that third molar. My question is, is it ok to extract that third molar in this age? or there is any other alternatives to save that tooth. If you need the x-ray to answer specifically I can scan it and send it to you.   • Hasib, In case of acute infection of wisdom tooth, local anesthesia is not very effective and more of anesthesia is required to numb the tooth. In case of acute wisdom tooth infection, antibiotics are given to control the infection and then extraction can be done. After course of antibiotics is completed, you can visit your dentist to get wisdom tooth extraction done. You can also get extraction done under conscious sedation so that there is no pain. After tooth extraction, foci of infection will be removed and pain will subside completely. You can visit an oral surgeon for extraction. By the time, you get tooth extraction done, avoid eating from that side of the mouth and keep wisdom tooth area clean. Food accumulates in wisdom tooth area will aggravate the pain.

  • Recently I went to a dentist to extract my right side lower jaw third molar tooth. The tooth has carriage in one side. But unfortunately he couldn’t numb the tooth due to my toothache although he tried a lot by giving three times more anesthesia. So later he told me that unless the pain subsides he won’t be able to extract the tooth. He also gave me antibiotics and said if the pain doesn’t subside he may give me some blood test also. Now this has become a critical situation for me. I have finished my antibiotic course but still the pain is there. When I take my meal I use cotton on carriage side of the tooth so that no food particle can enter in the tooth blood vessel. Please advise me what to do in this situation and what really happened to my teeth.

  • There is no harm in getting the wisdom tooth extracted at age of 40 years. Wisdom teeth don’t aid in mastication and are susceptible to infections so no treatment is required for them if they get infected. If wisdom teeth are infected or cause any problem, then they are pulled out. Wisdom teeth are last teeth to erupt in mouth and it is difficult to clean till the wisdom tooth so they are more susceptible to infections. After clinical and radiographic examination, your dentist will determine whether to perform simple tooth extraction or surgical tooth extraction for your wisdom tooth. In case of simple tooth extraction, after numbing the tooth, forceps and elevators are used to remove the tooth and in case of surgical tooth extraction, cut is given on gums and tooth is removed after bone cutting and or tooth sectioning. By the time, you get your tooth extracted; you can take some pain meds for some pain relief. Keep the wisdom tooth area clean.
